If you need to request a consult for your patient to be seen by a UVA specialist, please print and fill out the Request for Consult form and fax it back to 434.244.7521. Remember to fax all of the supporting documentation to ensure prompt scheduling: Patient Registration Information, Telemedicine Consent, Financial Screening, HIPAA, Green Card (Long Term Service Agreement), and any lab work or other supporting medical documents.

In case you need help using your videoconferencing system, here is a one page how-to guide. If you find that you need more one-on-one help, please call us to schedule a training session. We will videoconference with you and walk you through the process of how to use your equipment.

How to Join the UVA Telemedicine Network
You can become part of one of the largest telemedicine networks in the country. Telemedicine consultations are a cost effective way for your patients to receive specialty health care from UVA physicians without having to make the often long and costly trip to the UVA Medical Center in Charlottesville. If you would like to offer telemedicine services at your clinic, please contact us at telemedicine@virginia.edu. We will make the proper arrangements and help guide you in the process of becoming a UVA Telemedicine Partner Site. We will work hand-in-hand with you for the start-up process and to assist you with any questions you may have. Our staff looks forward to working with you and your patients.
