UVA-WorkMed is pleased to partner with UVa Human Resources offering health and wellness programs to team members and spouses who are enrolled in the University of Virginia Health Plan. Hoo's Well is the University of Virginia's comprehensive wellness program. It is designed to help you get healthy and stay healthy by improving your general fitness and reducing your risk for heart disease, cancer, diabetes, and other debilitating illnesses. Take part in taking care of yourself. Participate in the classes offered through this program to help you lose weight, improve your diet and nutrition, manage stress, and enhance your general health. The UVa Biometric Screening Event has concluded for 2014-15.
Hoo's Well Biometric Screening To know more about your state of health, it’s important to know your “numbers.” Why is this important? Your numbers can indicate whether or not you have a set of five risk factors that, in combination, can increase your risk for serious health problems such as heart disease and Type 2 diabetes. In the biometric screening, your height, weight, waist circumference, blood pressure, cholesterol values, and blood glucose will be measured. Body mass index will be calculated. This is a 10-minute screening and a 10-minute coaching session which will fully explain your results, as well as the programs available for you. The screening results are provided to you on a simple form that you can share with your Primary Care Provider.

Participants have three options to complete the biometric screening (choose ONLY ONE):
1. Schedule an appointment at one of the screening locations.
2. Schedule an appointment with your physician and use the Physicians Screening Form (no physician signature required). Fax a copy of your results and the Physicians Screening Form to 248-864-4409 no later than October 31, 2014. All participants are required to complete the Health Assessment on www.Aetna.com in order to receive the $480 premium savings.
3. If you've had blood work in the 2014 calendar year with a UVA physician, use the Physician Screening Form and a copy of your results located in "My Chart." Fax a copy of your results and the Physicians Screening Form to 248-864-4409 no later than October 31, 2014. All participants are required to complete the Health Assessment on www.Aetna.com in order to receive the $480 premium savings.
Be sure to add your own weight, height, blood pressure, and waist circumference to the Physician Screening Form.
