UVA-WorkMed is seeking an experienced RN for the newly-created position of Chronic Care Coach. The role focuses on assisting UVA employees and family members manage chronic health conditions such as hypertension, heart disease, diabetes, and morbid obesity. Specifically, the RN will provide personal health coaching, will facilitate utilization of Aetna’s disease management programs where indicated, will facilitate utilization of Hoo’s Well programs where indicated, and will facilitate the utilization of the full spectrum of Health System resources, as needed. The ultimate goal is to assist the employee or family member be has healthy as they can be. The successful candidate will have significant experience within the Health System, possess experience in disease management, as well as experience motivating patients to identify, set and meet health care goals. UVA-WorkMed is a comprehensive occupational health and employee wellness clinic within the University of Virginia Medical Center. We partner with corporations, businesses, governmental agencies, and nonprofit entities to provide occupational health and wellness services that are tailored to the needs of the employer. UVA-WorkMed also provides medical treatment of work-related injuries. Our Medical Director, David S. Rubendall, D.O., specializes in the treatment and medical management of work-related injuries. The UVA-WorkMed philosophy is to provide treatment as quickly as possible, expedite referrals to specialists when indicated, and facilitate the return-to-work process. We recognize that the longer the employee is off work, the poorer the prognosis for a full return to work.
UVA-WorkMed staff members include Registered Nurses, Licensed Practical Nurses, Exercise Physiologists, Registered Dietitians, and Wellness Specialists. Our staff has more than 50 years collective experience in the delivery of occupational health and wellness services.
