  • Online survey administered June 12 - 26
  • A link to take the survey, containing a unique password, will be distributed via email to each employee on June 12th
    • DO NOT forward
  • Average completion time – 10 minutes
  • Once survey is submitted it goes directly to survey vendor
  • Response rates provided daily throughout the survey – Goal is 90%
