Eligibility for Case Management

Regardless of income (high or low), Case Management services are available to any child who is:

·         Under the age of 21

·         Lives in Virginia

·         Has a diagnosed physical disorder that is expected to last longer than 12 months.

Examples of qualifying conditions include (but are not limited to): diabetes, cystic fibrosis, muscular dystrophy, deafness or hard of hearing.

Case Management services are free of charge.
