Please come up to the receptionist's window when you arrive
at the Teen Health Center. You'll be asked for information such as your
address, insurance(s) (bring any insurance cards you have), the person
who is responsible for your bill, and emergency contacts. If your parents
or legal guardian does not know you are here, be sure to discuss this
with our receptionist.
