How To Schedule an Examination
or Procedure for Angio-Interventional:
- Call Angio-Interventional Radiology at (434)-924-9401 (outside) or 4-9401 from inside of hospital.
- Let the receptionist know what type of procedure you would like to schedule, what day and a time frame if needed.
- The receptionist will need the following information:
- A written request or a consult form if the patient is an outpatient (which may be faxed to us at 434-982-0887), or a MIS order if the patient is an inpatient.
- Current lab work (PT, PTT, Chem and CBC w/plt) if the patient had labs at another facility they can be faxed to 434-982-0887. Call Angio at 434-924-9401 for questions or concerns.
- An office note or a work-up on the patient (may be faxed – if faxed, please send to 434-982-0887).
- For most angiograms/arteriograms the patient will need a 23hr stay in the hospital, this should be set up by the referring service. If the referring physician does not have admitting privileges at UVA, please call Angio at 434-924-9401 so that we may arrange admission for this patient.
