Guidelines
1. “Observation” Student to observe in a Non-Clinical/Non-Patient Care Setting:
- Contract/Agreement:
Complete a Medical Center Observation Agreement or Observation Affiliation Agreement obtained from Medical Center Contracts Management. (924-5039)
- Medical Center Orientation:
Review orientation information as provided in the Medical Center Observation Agreement. Obtain required signatures.
- Infection Control:
Review infection control training information as provided in the Medical Center Observation Agreement. Obtain required signatures.
- Criminal Background History:
No Disclosure Required
- Identification:
All students and instructors accompanying students must obtain a UVA Health System Photo Identification Card. UVAHS Clinical Coordinators will make arrangements for students to obtain ID badges. (ID Badge Office number 982-4099)
2. “Observation” Student to observe in a Clinical/Patient Care Area (no hands on care)
A. Observation Student observing for < 4 weeks in a clinical/patient care area
- Contract/Agreement:
Complete a Medical Center Observation Agreement or Observation Affiliation Agreement obtained from Medical Center Contracts Management (924-5039)
- Medical Center Orientation:
Review orientation information as provided in the Medical Center Observation Agreement and obtain required signatures
- Infection Control:
Review infection control training information as provided in the Medical Center Observation Agreement. Obtain required signatures.
- Criminal Background History
No Disclosure Required
- Identification:
All students and instructors accompanying students must obtain a UVA Health System Photo Identification Card. UVAHS Clinical Coordinators will make arrangements for students to obtain ID badges. (ID Badge Office number 982-4009).
B. Observation Student observing for > 4 weeks in a clinical/patient care area
- Contract/Agreement:
Complete a Medical Center Observation Agreement or Observation Affiliation Agreement obtained from Medical Center Contracts Management (924-5039)
- Medical Center Orientation:
a)Standard Precautions/Infection Control: Student or Referring Facility must provide documentation of Standard Precautions training within past 12 months. If documentation not available, student must complete the UVA Standard Precaution/Infection Control Computer Base Learning Module.
b)Complete the Medical Center Orientation Mandatory Training. Contact the Center for Organizational Development (924-2502 or email “hrcod@virginia.edu”) to register student/instructor for mandatory training. Additional orientation training may be required as determined by managers based on work environment.
- Infectious Disease:
Students and instructors (or their referring facility) must provide documentation of a TB test within the past 12 months and appropriate care if positive. Students (or their referring facility) must provide documentation of appropriate vaccination against measles, mumps, rubella (requires 2 doses of MMR after first birthday for those born after 1956), diphtheria, and varicella, or have satisfactorily proven immunity to these diseases.
- Criminal Background History
No Disclosure Required
- Identification:

3. “Participation” Student in Delivery of Direct Patient Care, Treatment, or Services
- Contract/Agreement:
Complete a Medical Center “Participation in Delivery of Healthcare Services” Affiliation Agreement obtained from Medical Center Contracts Management (924-5039).
The Discipline (OT/PT/RT) Student Placement Coordinators are required to verify current contract status (or establish a new contract) with Contract Management (Cindy Baber 924-5039) prior to accepting a student.
PT and OT second affiliation students are preferred, but first affiliation students will be considered on occasion for exceptional candidates.
- Medical Center Orientation:
a)Standard Precautions/Infection Control: Student (or their Referring Facility) must provide documentation of the student’s Standard Precautions training, and training in TB prevention measures within the past 12 months. If documentation is not available, the student must complete the UVA Standard Precaution/Infection Control Computer Based Learning Module prior to working in clinical areas.
b)Complete the Medical Center Orientation Mandatory Training. Contact the Center for Organizational Development (924-2502 or email “hrcod@virginia.edu”) to register the student/instructor for mandatory training. Additional orientation training may be required as determined by managers based on work environment.
c)All therapy students/instructors must receive Health System orientation within the first week of their training. The Student Placement Coordinator will schedule this orientation with the student. Students are expected to begin their affiliation with the Health System to coincide with new employee orientation which starts on alternate Mondays – check with the Student Placement Coordinator for dates. Special arrangements for starting on alternate dates must be made in advance with the Student Placement Coordinator.
d)Mandatory Net Learning Modules are assigned to All Students/Instructors regardless of length of stay: (See 2005 Accessing Net Learning)Rebecca Combs (924-8390) enters students into Net Learning.
e) CPR Certification is highly recommended for therapy students.
- Infectious Disease:
Students and instructors (or their referring facility) must provide documentation of a TB test within the past 12 months and appropriate care if positive. Students and instructors (or their referring facility) must provide documentation of completion of hepatitis B vaccination series by the end of the student/instructor’s first clinical rotation in the Health System. Students (or their referring facility) must provide documentation of appropriate vaccination against measles, mumps, rubella (requires 2 doses of MMR after first birthday for those born after 1956), diphtheria, and varicella, or have satisfactorily proven immunity to these diseases.
- Criminal Background History
College/Facility must obtain disclosure of criminal background history from Student. College/Facility must certify that student has reported no relevant criminal background history and/or that a criminal background check has revealed no relevant criminal history information.
- Identification
All students and instructors accompanying students must obtain a UVA Health System Photo Identification Card. UVAHS Clinical Coordinators will make arrangements for students to obtain ID badges. (ID Badge Office number 982-4009).
- Parking:
Students are responsible for contacting Parking and Transportation prior to their first day of orientation to obtain a temporary parking pass. Passes are available for purchase for the U-Hall parking lot. (Contact Parking and Transportation 924-7231)
- Housing
It is the student’s responsibility to identify housing for the time they are affiliated with UVA Health System.
