Drug Information Practice Residency:
Application Requirements

The applicant must be a highly motivated individual who desires to obtain advanced education and training leading to an enhanced level of professional practice.

1.

The applicant must be enrolled in (or be a graduate of) an advanced pharmacy degree program or have equivalent experience and be eligible for licensure in Virginia. Pharmacy practice residency training is highly recommended.

2.

The applicant should have some prior hospital pharmacy experience.

3.

The applicant must submit the following information by the specified deadline:

 

a.

application form

b.

letter of intent

c.

academic transcripts

d.

curriculum vitae

e.

three letters of reference from instructors, rotation preceptors, or previous employers (instruction forms should be submitted with letters)

f.

two samples of technical writing (papers, newsletter articles, presentation handouts, etc.)

4.

An on-site interview with the residency program directors, Director of Pharmacy Services, selected department administrators, and clinical pharmacy specialists is required.
