The applicant must be enrolled in (or be a graduate of) an advanced pharmacy degree program. 

The applicant must be eligible for licensure in Virginia.

Prior hospital pharmacy experience is strongly encouraged.

4.

The applicant must submit the following information by the specified deadline:

Letter of intent

Application form

Official pharmacy school transcript

Three letters of reference from instructors, rotation preceptors, or previous employers (instruction forms should be submitted with letters)

5.

An on-site interview with the residency program directors, Director of Pharmacy Services, selected department administrators, and clinical pharmacy specialists is required.
