Before Your Appointment

All new patients and those who have not been seen at UVa in more than a year are asked to complete a health questionnaire prior to their appointment.  The questionnaire will be sent to you by mail.  Please complete it and bring it with you for your appointment.  The questionnaire provides the orthopaedic surgeon with important information about your current orthopaedic problems and your prior medical care.

A staff member may contact you by telephone prior to your appointment to review your health insurance benefits with you.  This staff member may advise you of managed care referral requirements that must be met to maximize your  health benefit coverage and to minimize your out-of-pocket expense.  This person is your advocate in helping you understand what your health insurance will cover.

Please arrive 15 minutes before your scheduled appointment time and bring information about all of your insurance policies and policy numbers.
