Appointment Information
How to Prepare
- Fill out the Medical History form and give it to the Reception Desk when you arrive in the Eye Clinic. If you have a list of your medications you can attach it to the Medical History form.
- Bring any insurance cards to your appointment and give them to the staff when you register. We will bill Medicare or your insurance after your visit. If you are a member of a Managed Health Care Plan, please obtain any needed referrals before your appointment. If your insurance requires a co-pay to see a specialist, that co-pay is due at the time of registration. If you have any concerns about your ability to pay or method of payment that we accept, please let us know.
- If you wear glasses, please bring them to the appointment. If you wear contacts you should be prepared to have them removed during your exam.
- Write down a list of any questions you have for the doctor and give it to him during your appointment. We are happy to address any concerns you might have about your eyes while you are here.

What to Expect
- Plan for a 2 - 3 hour appointment, especially if this is your first visit. If you are diabetic please be sure to bring a snack with you.
- Both of your eyes will be dilated during your exam. If you have concerns about driving while your eyes are dilated, please bring a driver to help you home.
- The University of Virginia is a teaching hospital. You may be examined by a resident, a fellow, or a technician before an attending examines you. The residents and fellow are licensed physicians who have completed their medical training and are studying sub-specialty techniques in in our clinic. The technicians are specifically trained in opthalmic examinations.
- We may check your glasses and perform a refraction during your visit, but we are not able to write glasses prescriptions for our patients. Obtain glasses prescription from your referring/general ophthalmologist or an optometrist.
- We will send a copy of our consultation notes to your referring physician. If you would like this information sent to other doctors please have their names and addresses available.
