Animal Protocol Medical Evaluation

-Nursing review of completed questionnaire
-Clinical evaluation for any problems and needs identified

Blood and Body Fluid Exposure Evaluation
Services offered include:
-Nursing evaluation of exposure site and source information
-Laboratory baseline screening
-Physician evaluation for possible post-exposure prophylaxis if high-
risk exposure
-Counseling regarding exposure risk and prevention of exposure to
others
-Followup laboratory screening
-Secure retention of medical record
CPR Classes
The goal of every CPR class is to fight heart disease and strokes.
Services offered include:
-CPR – Family and friends (adult, infant & child) your choice
-Heart Saver (adult, infant and child) your choice
-Heart Saver and AED (adult with Automatic External Defibulator)
-Basic Life Support for Health care Providers (adult, infant
and child plus AED) a must course for potential instructors
-Instructor CPR
Environmental Exposure Evaluation
Services offered include:
-Nursing evaluation of exposure circumstances and medical status
-Clinician evaluation if indicated for severity of symptoms or need for
medical treatment; Toxicology consultation if necessary
-Notification of appropriate situation-evaluating agency, such as
Environmental Health and Safety documentation of exposure and evaluation
notification to employer of work status if indicated
-Secure retention of medical record
Ergonomic/Worksite Evaluation referrals
Fitness for Duty
Services offered include:
-Physician review of medical history and therapies with employee
-Physical examination as indicated
-Physician collection and review of work recommendations from primary care
providers
-Written communication of work recommendations to employer and employee

Fitness for Work
Services offered include:
-Nursing assessment of medical condition or symptoms
-Clinician evaluation of employee if necessary
-Initiation of followup evaluation if urgent
-Notification to employer of work status

Hearing Protection Evaluation

Immunizations including Influenza and Hepatitis B vaccine
Services offered include:
-Nursing review of health history, immunization status, contraindications to
vaccination
-Immunization administration (hepatitis B, tetanus, varicella, rubella, rabies,
and influenza) and follow-up as indicated (titers, exams for reactions,etc.)
-Documentation of immunization status
-Notification to employer of work status if indicated
-Secure retention of medical record
Infectious Disease Exposure Evaluation
Services offered include:
-Nursing contact of individual exposed
-Nursing assessment of exposure
-Clinician treatment for necessary prophylaxis if work-related exposure
-Referral to primary care provider for treatment of non-work-related
exposure
-Notification to employer of work status if indicated
-Secure retention of medical record

OSHA/Hazardous Material Surveillance Evaluation
Services offered include:
-Medical history review
-Occupational history/ exposure review
-Completion/review of appropriate questionnaires for specific hazardous
material
-Physical examination
-Laboratory testing as indicated
-Special testing as required, including pulmonary function testing,
audiometric testing, electrocardiography
-Immunizations as indicated
-Written recommendations to employer regarding work status
-Written communication to employee, with review of findings of evaluation
and recommendations
-Secure retention of medical record
Health Risk Appraisal (HRA)
-The HRA Questionnaire is a Personal Wellness Profile used to help identify certain cardiovascular health risks as it relates to family history, lifestyle habits and mental health. In conjunction with the questionnaire, physical measurements such as blood pressure, blood cholesterol levels, glucose level and body mass index are factored in to generate individual Personal Wellness Profiles.
-The HRA participant receives updated health risk information which can be used to make lifestyle changes and take control of managing their own health. It further allows them to share this information with their PCP and become better partners with him/her.
-Prior to Screening Day: Participant makes an appointment for their screening and the questionnaire will be mailed to the participant for completion. The questionnaire must be brought with them on the day of the screening. The physical measurements take approximately 15-20 minutes and include the following:
Height
Weight
Blood lipid values, including cholesterol
Blood Pressure
Risk Reduction Programs
Services offered include:
-Executive risk reduction program
-Cardiovascular program
-Company specific programs
-Nutritional counseling
-Exercise/weight control program
-Other requests: if services cannot be provided through Occupational
Health Services, the customer will be assisted in procuring the necessary
services through sources of service at the University of Virginia or in the
community.
Routine Physical Exams required for work
Tuberculosis Screening
Services offered include:
-Nursing review of PPD status and history
-Administration of PPD
-PPD reading at 48-72 hours, and recording of results
-Evaluation of positive PPD with symptom review and chest radiograph
-Physician evaluation for discussion of positive PPD and consideration of
prophylaxis or medical treatment
-Notification to employer of work status if indicated
-Secure retention of medical record
Worker’s Compensation injury evaluation and care
Services offered include:
-Completion of report of injury/illness and choice from panel of physicians
-Medical evaluation and examination
-Written communication of work restrictions and follow-up appointment date
-Referral for specialized evaluation or additional treatment as necessary,
with followup in Occupational Health Services
-Completion of all injury/illness/treatment documentation
-Timely transfer of documentation information to Worker’s Compensation
management team
-Secure retention of medical record
