Animal Handler Instructions

All animal handlers are required by federal regulations to have an occupational health evaluation prior to working with animals and to have a re-evaluation on an annual basis thereafter.  (Some conditions, such as working with non-human primates, require more frequent health assessments.)  Employees are to report to UVA-WorkMed and students are to report to Student Health to complete this requirement.  Failure to report for the preliminary evaluation or subsequent re-evaluations will result in the loss of privileges to work with animals and the revocation of protocol approval.  A record of your visit to UVA-WorkMed or Student Health will be made electronically.

Instructions for completing your Occupational Health visit:

1.  Schedule your occupational health evaluation appointment:.

Employees:   Call UVA-WorkMed to make an appointment for your evaluation. Phone:  243 -0075.  (Or email: uvaworkmed@virginia.edu )   Location:  Go to Fontaine Research Park, enter building 515, take a right, take the elevators to the third floor and take a left off the elevators. UVA-WorkMed is on the left, Suite 3200.

Students:  Call Student Health to make an appointment for your evaluation.
Phone:  982-3915.  Location:  Elson Student Health Building (Corner of JPA & Brandon Ave.)

2.  Download Occupational Health form from the ACUC on-line protocol website:  https://vprgsecure.web.virginia.edu/acuc/.  (You must be listed as an animal handler on the protocol before the form is available.)

A.  Click on Animal Handler Access.

B.  Enter your last name and Social Security Number (without spaces or dashes) and select Get Animal Handler Profile.

C.  The screen will now list your name and current health status.  Click on  Get Occupational Health Form.

D.  Print out the form, fill it in, and take it with you to your appointment.  Be sure that your supervisor/PI signs on the designated line before you go. Also, please be sure the PTAO number is filled in as this is mandatory for billing purposes. WorkMed is unable to see you without that number.

3.  Bring with you to UVA-WorkMed (employees) or Student Health (student):

     A.  The completed Occupational Health form SIGNED BY YOUR SUPERVISOR/PI.

     B.  Any information you have regarding dates of past vaccinations (tetanus, rubeola
laboratory testing, skin testing for tuberculosis, hepatitis A, hepatitis B).

NOTE:  Expect to spend approximately 15- 20 minutes for your evaluation.  Free parking is available.
