Download the CNS Committee Survey
Please take the time to download, fill out, and email this survey to rsr6f@virginia.edu
- Click on the "Download the CNS Committee Survey" link.
- Select "Save this file to disk" and remember the directory to which you save it.
- When the download is complete, either click "Open" or use Windows Explorer to find the file on your PC and double-click to open it in Adobe Reader.
- Respond to the questions (when you point to a field in which data can be entered, you will see the cursor change from an open hand to a pointing finger).
- When finished, save the file.
- Send the file to me at rsr6f@virginia.edu
If you do not recall whether you have already filled the survery out, please e-mail me at rsr6f@virginia.edu and I will tell you whether a form is on file. Feel free to email me with any other question related to this project.
