What to Bring
If at all possible, please send copies of your pathology slides (available through your hospital's Pathology Department) a few days before your appointment date. We will also need a copy of the pathology report. This report may be available from your hospital's Medical Records Department or from your physician. Note: Even though you may be charged a fee for copies, do not send originals.

If you are not able to send us anything ahead of time, please bring the following to your appointment:
- copies of your pathology slides (available through your hospital's Pathology Department);
- copies of your MRI and any other films such at CAT scans, x-rays, etc. (available through your hospital's Radiology Department);
- doctors' notes and reports from your medical record (generally available either through your physician's office or, for hospitalizations, through your hospital's Medical Records Department).
