INPATIENT PHLEBOTOMY SERVICES

HOURS: Routine phlebotomy services are provided every two hours, 24/7.
All routine laboratory orders for the 06:00 collection by the Phlebotomy team must be entered into the Medical Information System (MIS) by 03:25. All lab orders must be entered into MIS prior to the scheduled collection time in order for those specimens to be drawn during that specified collection time. STAT orders are the responsibility of the unit.
Blood collection by the Phlebotomy Team will include these schedules:
- ROUTINE (such as "Today")
- TIMED (orders entered for specific times such as "Today at 18:00")
Between 22:00 and 03:25, any lab orders entered with the schedule of TODAY will be collected during the morning 6 am rounds.
If the patient has a pending discharge, labs should be ordered in the Discharge Pathway via MIS.
OUTPATIENT PHLEBOTOMY SERVICES
Outpatient phlebotomy services are available at several locations throughout the Medical Center and at the Northridge Facility. All patients who present at one of these locations for specimen collections must be registered into the Medical Center Patient Access system. Specimen collection will be performed on patients only at the written or electronic request of a physician or authorized individual. See page 7 for directions for completing lab request forms. Oral requests for collection of laboratory tests cannot be honored unless accompanied by a written (FAX copy is acceptable) or electronic request.

SPECIMEN TRANSPORTATION
Specimen Transport Within Facility: https://www.healthsystem.virginia.edu/intranet/tubesystem/
- Remove all needles and cap syringes
- Assure all tubes are labeled correctly
- Assure specimen temperature requirements are met (i.e. Lactate on ice).
- Tighten all specimen container lids.
- Place labeled specimens in clear zip-lock plastic bag and seal bag.
- Place transmittal or consult form in the outside pouch of the clear zip-lock bag.
- Place bagged specimens into large clear zip-lock pastic bag and seal.
- Specimens that cannot be transported via Pneumatic Tube System are delivered directly (i.e. Leukin Traps with tubing attached, 24 hour urine containers).
Specimen Transport Outside Facility - Delivered by Courier
- Remove all needles and cap syringes
- Assure all tubes are labeled correctly
- Assure specimen temperature requirements are met (i.e. Lactate on ice).
- Tighten all specimen container lids.
- Place labeled specimens in clear zip-lock plastic bag and seal bag.
- Place requisition form in the outside pouch of the clear zip-lock bag.
- Place properly bagged specimens in a rigid, leakproof transport unit which maintains temperature requirements.
- Deliver specimens to Laboratory immediately.
Specimen Transport via U.S. Mail Services
- Remove all needles and cap syringes
- Assure all tubes are labeled correctly
- Assure specimen temperature requirements are met (i.e. Lactate on ice).
- Tighten all specimen container lids.
- Place labeled specimens in clear zip-lock plastic bag and seal bag.
- Place requisition form in the outside pouch of the clear zip-lock bag.
- Place properly bagged specimens in a rigid, leakproof secondary container with absorbent material and seal.
- Prepare specimens for shipping to maintain temperature requirements.
- Place secondary sealed container in a shipping box.
- Address mailer to:
