BLOOD BANK SPECIMEN COLLECTION- TYPENEX® IDENTIFICATION PROCEDURE
Specimens submitted to the Blood Bank for type and screen and intended for compatibility testing must be labeled using the Typenex Blood Recipient Identification System. Arm bands are generally available in patient care areas where phlebotomy supplies are stored.
PROCEDURE:
1. Verify the patient's identity by asking him/her to state their name. Match patient name and history number that appears on the patient's hospital arm band and MIS transmittal. The middle name is not required, but Jr., III, etc. are required. All information must agree before proceeding.
2. If patient is wearing an existing Typenex band, check to see if the corresponding sample is still available for use before proceeding. If the dating has exceeded it's 3 day limitation, cut the band off and proceed.
3. Collect the patient blood specimen into a 6 ml EDTA (pink top) tube according to appropriate procedure.
4. Using a ball point pen, legibly write the patient's full name, including suffix (Jr, Sr, etc...), history number, date of specimen collection and the phlebotomist's first initial and last name on the long red label.
5. Remove the long red label from the arm band and label the patient sample tube with the alpha-numeric Typenex number at the top of the tube.
6. Remove the protective backing that holds the strip of extra numbers from the back of the arm band. Affix the strip of numbers to the specimen collection tube.
7. Wrap the Typenex arm band once around the patient's wrist or ankle, so that the numbered side is out and the clear white area lies between the front and rear guides of the bracelet clip.
8. Firmly close the bracelet clasp.
9. Remove the coded self-sticking number from the Typenex arm band and attach it to the MIS transmittal to accompany the sample.
