Introduction

UVa Health System and University of Virginia faculty and staff may authorize one or more Designated Individuals to borrow materials from the Health Sciences Library on their behalf. Only individuals affiliated with the University may be Authorized Borrowers. The authorization is good for one year, unless otherwise specified.

Step-By-Step

  1. Pick up an Authorized Borrower form from the Circulation Desk at the Health Sciences Library, or download the PDF.
  2. Complete the Authorized Borrower form, and attach a photocopy of your ID.
  3. Return the form to the Circulation Desk, or fax it to (434) 982-4238.
  4. Remind your Designated Individuals to present their ID's at the time of checkout.
