Inpatient Schedules & Curriculum

Ward Team Structure

roundsEach inpatient team consists of an attending physician, two residents, two interns and one to two medical students. Residents and interns are paired 1:1. There is typically a fellow on the subspecialty rotations but not on the general medicine services. The intern is the patient's primary physician under the mentorship of the resident and attending.

Call Structure

Each ward rotation consists of 3 to 5 resident/intern/medical student teams. During a standard month, each team spends three weeks on day-call and one week on "night float" taking overnight admissions.

During day call, patients are admitted from 7:30A until 7:00P. The night float resident and intern arrrive at 8:00P. After sign-out to the night float team, the day team goes home by 9:00P. Night float obligations are Sunday through Thursday nights, with Friday and Saturday nights covered by an off-service resident.

UVA's day/night float model not only meets the national Residency Review Committee mandates for work hours, but also allows integration of the night float team into morning attending rounds. This has dramatically improved our patient care continuity and resident education. Typical inpatient rotation schedules are outlined below.

Ambulatory Schedules & Curricula

There are three types of outpatient/ambulatory blocks at UVA:

  • conventional electives
  • ambulatory blocks
  • “specials” blocks

Conventional electives may involve both outpatient care and inpatient consultative experiences, usually in the medical subspecialties. During ambulatory blocks, which are exclusively outpatient, residents participate in clinical experiences and a structured ambulatory curriculum that progresses through the three years of training (see template below). To ensure that all residents receive the same curriculum, they are scheduled for at least one ambulatory block in the fall and one in the spring of each year. Specials blocks are designed to allow protected time for resident research projects. Residents are also encouraged to use elective time for international rotations.
