Requirements for Housestaff Reappointment


Clinical Department Responsibilities

1. Housestaff Reappointment Recommendation Form (replaces previous letter from clinical department chair):

  • Complete all requested information
  • Signature of clinical department chair required
  • Dates of appointment - limited to one year only

2. Supplemental Application for Housestaff Reappointment (replaces previous Housestaff reappointment form), completed by the applicant:

  • All questions must be completed
  • For any questions responded to with YES (except #1a), additional documentation must be provided as specified
  • For YES responses to question 9:  Practitioners who are or have been defendants in medical malpractice actions must submit a letter from the attorneys who are/were assigned to represent the practitioner in the malpractice actions. The letter must address:
    • current status of malpractice case
    • practitioner's role in alleged negligent actions
    • whether expert reviews of the practitioner's care have been obtained and the general substance of those reviews
    • anticipated trial date or expection of settlement
  • For YES responses to question 12: Practitioners who have privileges at other institutions must provide verification of their status and standing at those institutions. The General Reference Form may be used to secure this information.
  • Must include original signature of applicant

3. Annual Evaluation of Applicant

  • Hospital Evaluation Forms will be provided for each resident/fellow currently enrolled
  • An evaluation must be provided with the reappointment request. As in the past, the department may use the Medical Center form or provide copies of departmental evaluation forms.
  • PLEASE NOTE: In cases where the current department will not be responsible for reappointing the resident, the evaluation form(s) should be forwarded directly to the GME Office. Be aware that a delay in forwarding these forms will delay reappointment of the resident.
    Departments who are reappointing residents currently appointed in another department should submit all documents required for reappointment EXCEPT the evaluation form(s). The GME Office will be responsible for collecting these and appending them to the reappointment application.

4. Proof of Virginia licensure, temporary or permanent: Must be a copy of the current wallet-sized license indicating expiration date.

5. Secure transcript reflecting completion of required training modules (completed on-line through NetLearning Web Education) and proof of completion of other required instruction (Universal Precautions, Health System Compliance). See Mandatory Training Requirements.


Graduate Medical Education Office Responsibilities

6. Secure evaluation forms for applicants changing departments.

7. Secure malpractice claims history from Office of Risk Management.

8. Renew temporary Virginia medical licenses.

9. Query the Department of Health and Human Services Office of Inspector General (OIG) website.

10. Query the General Services Administration (GSA) website.
