General Information
All medical, dental, and clinical psychology housestaff must be appropriately credentialed and appointed to the Professional Staff of the University of Virginia Health System, in accordance with the following requirements, prior to assuming patient care responsibilities.
Appointments are made for one year and are renewed annually on the recommendation of the Clinical Chair. Offers for initial appointment and reappointment are contingent upon credentialing and privileging by the institution and subject to review and approval of all materials submitted in the application process. Such appointments are reviewed and approved by the Health System's Credentials Committee and the Medical Policy Council. Specific requirements for appointment can be obtained from the Medical Staff and Residency Office which is responsible for obtaining and verifying credentials information related to housestaff and for maintaining files on current and past housestaff.
As part of the credentials process, queries are made to the National Practitioner Data Bank for all new applicants who do not come through the National Residency Matching Program. In addition, at the time of initial appointment and reappointment, queries will be made to websites maintained by the Health and Human Services Office of the Inspector General and the General Services Administration.
All required materials must be submitted to the Graduate Medical Education Office (Box 800136) no less than 90 days prior to the effective date of the housestaff member's appointment/reappointment.
The Credentials Coordinator will review the materials and:
- Submit them to the Credentials Committee for approval
OR - Notify the appropriate clinical department chair, or designee, of deficiencies which prevent approval (i.e., missing documents, copies vs. originals, etc.). The application will be submitted to the Credentials Committee as soon as the deficiency is corrected.
Upon approval by the Clinical Staff Executive Committee, an appointment letter will be sent by the Chair of the Credentials Committee to the Housestaff member. Acceptance of the appointment is required within 10 days.
It is the responsibility of the Clinical Department Chair to notify the Credentials Committee of any changes in a housestaff member's clinical privileges (additions, reductions, restrictions, termination, etc.). Notification must be submitted, in writing, when the change becomes effective as an amendment to the current appointment.
