Creating an Online Survey
Online surveys can be created to measure anything you want. They can be used to receive feedback from residency applicants about their experience on interview day, or assess how your training program prepared alumni in their various settings.
Have prepared your list of questions and the format for responses that you wish to receive. Are the questions Yes/No answers, can the target audience choose from a selection of responses, is the surveyee expected to enter text? Think about what would make you want to respond to a survey, and chances are that your target audience will react in the same way. Keep your list of questions short 5-6 is usually the amount that people are prepared to answer.
Keep the questions clear and concise. If they don't make sense, or are long and involved, your response rate will be very low.
Steps:
- Login to your website.
- Select Create a File.
- Answer points 1-3 on the page.
- Go to the bottom of the page and select Build a form. Return to the top of the page and select Create.
- Highlight the form, and select Edit File.
- Work through each screen. You have the option to either use the default questions or create your own questions.
- Select, according to your list how you wish the responses to be identified - drop down menu, radio buttons, text response.
- You can Save and Close at any time. It allows you to view the survey in the same way as your end user. To go back into the form just click on the Back button on your browser. You are also able to move questions up or down before publishing.
- Once you have completed creating your survey, you will need to publish it.
- Go to your maintenance screen, and select Publishing.
- Check the box for your survey and then select Publish at the top left corner of the page.
- You can send the survey to yourself as a test before publishing so that you can become the end user.
- Send the URL (web survey address) to the end user group, advising them that the survey is anonymous, the importantance of their input to the continued growth of your program. If people know that they cannot be identified, they are more likely to complete it and be candid.
