For the User Departments

User Request Form 
User departments must complete a User Change Request form (UCR) for any desired changes to the construction project.  The UCR must be approved by your department's respective "chief" prior to submission to the Project Manager, Wendy Steesy for consideration.  Each request will be review by the project team for cost and schedule impacts and the final decision will be made by Health System Administration.  No user requested changes will be accepted 60 days or less prior to the planned occupancy date of an area or within the first 30 days of occupancy unless it is deemed "mission critical" and the department can not function without it.

Telephones:  The new building will have a new phone system which is running on a data platform rather than the analog system you have today.  To properly coordinate the transition to the new system the user department should request a "telephone consultation" for the departmental move to the Hospital Expansion Project on a TSR form submitted to ITC.  Please also include your planned move/occupancy date on the form.  This electronic form can be found at http://etg07.itc.virginia.edu/uvaforms/servlet/search/home by putting the word "telephone" into the search field.  This will bring you to the electronic TSR which is a Communications Services Request form CC40.  Once this form is received Siemens/Rolm will contact you to set up a meeting to review phones including: phone numbers which will move, new phone numbers required, how the new phone system and devices operate, and removal of the old phone devices.
