WARD FELLOW: 2 teams - 1 attending/1 fellow/2 resident-intern pairs/1 MSIII per team
- Responsible for overseeing the daily care of all patients on the hem/onc service by serving as consultant to house officers and liaison with faculty and staff.
- Pre-round on patients with residents/ address immediate problems as necessary.
- Attend Morning Attending Rounds.
- Manage specialized care and write chemo orders for inpatients.
- Perform all bone marrow aspirations/biopsies on the inpatient team.
- Share on-call fellow for the month responsibilities with the fellow on the other team. Every other night and every other weekend call – home call with return to hospital as needed.
- Teach on rounds and during scheduled fellow teaching sessions during didactic hour.
- Accept transfers from other services and other hospitals, facilitate admission of patients from clinic and arrange follow up for patients seen in the ER.
- Cover Stem Cell Transplant clinic at night and on weekends and holidays.
- Cover Hematology and Oncology consults 5PM-8AM and on weekends/holidays.
HEMATOLOGY CONSULTS: 1 Fellow, may have resident and/or medical student
- Take new heme consults 8AM-5PM , M-F.
- No on-call responsibilities once active patients on service are addressed.
- Staff all inpatient and urgent “unassigned” outpatient consults with heme consult attending and provide follow-up care or disposition.
- Perform, interpret and sign-off all bone marrow aspirations on heme consult patients.
ONCOLOGY CONSULTS: Shared by inpatient fellows, may have resident
- The inpatient fellows share oncology consults with coverage every other day and on nights, weekends, and holidays when they are also on inpatient call.
- Responsible for performing, interpreting, and signing-off bone marrows performed on consult patients.
- Consults limited to inpatients or rare urgent outpatient unassigned patients.
- Staff all consults with the oncology inpatient attending.
ELECTIVE:
- Required electives need to be completed during the fellowship. You elect when to do them. See list and description of required electives.
- Fellow designed electives require Goal, Objectives, and Description of Activityform.
- No call on elective.
- Ward service “back-up” duty shared with other fellows on elective. Must be available for ward duty if primary fellow unavailable. See “fellow coverage” policy.
- Must be available 8AM-5PM , M-F or appropriately signed out. See Duty Hours Policy.
CLINIC:
- Each fellow will have 2 half-day clinics per week: one continuity clinic that will continue for the entire fellowship and one faculty clinic that will rotate every 4 months.
- Morning clinics are scheduled from 8 am-12 pm and evening clinics are scheduled 1 pm-5 pm. Exceptions to start times:
- Ward fellow will report at 10 after completing inpatient rounds
- Required meetings/lectures/programs may delay start time by 30-60 min
- Clinic fellow responsible for bone marrows on clinic patients of preceptor and on their own clinic patients.
- First “call” for nurse coordinators assigned to preceptor’s clinic.
- Notify in writing (email) the clinic attending, nurse coordinator and secretary one-month in advance for planned absences. See vacation and leave policies.
- Primary coverage for clinic patients (calls, scripts, visits) when preceptor not available.
- Arrange coverage by another fellow when away from UVA.
- Must be available 8AM-5PM for coverage. See duty hours, vacation, leave policies.
CONFERENCES:
- Fellows must attend all required conferences (core lectures, Journal Club, Grand Rounds, etc.) unless emergent patient issue interferes.
- Must attend one tumor board a week, preferably for preceptor’s specialty unless schedule conflicts with primary service obligation. Encouraged to attend other tumor boards especially while on elective.
- Required to present 2-3 Journal clubs and at least one Grand Rounds per year.
CALL:
- On-call fellow is responsible for all inpatient and outpatient calls, urgent consults and admissions. See Duty hours and on-call responsibilities policy.
- When covering weekday and weekend call, the covering fellow must make an effort to be available by 5pm . Check call schedule – know your assignments.
- Cover all consults, hematology and oncology, on the weekend with ward attending.
- Ward fellow is responsible for holiday call (M-F) – exceptions will be on calendar.
MISC:
- Fellows must take an active role in clinic patient management by meeting with preceptor to discuss scans, labs, and clinical course of patients between clinic visits.
- For Fellows Clinic patients, all needs of the patient are the primary responsibility of the fellow.
- Consult fellows must arrange disposition for all consult patients and communicate with physician who will assume care. May include returning patient to primary MD, referring to local specialist, or referring to “disease specific” clinic at UVA.
- Coverage must be arranged for any time away. (STRONGLY discouraged during consult or ward months unless to regional or national meetings.)
- A research plan is due by the beginning of the second year for clinical or basic track.
