Blastic Variant of Mantle Cell Lymphoma
Mantle cell leukemia (blastic variant): Peripheral blood smear, Wright-Giemsa stain, 1000x
Description:
In this aggressive variant of mantle cell lymphoma/leukemia the blasts resemble L2 ALLs in size. The cytoplasm is variable in amount and grey to blue-grey in color. The nuclear appearance is variable with frequent clefts present. The nuclear chromatin is immature and evenly dispersed with variable clumping, and nucleoli are usually visible.
Immunophenotype:
C19+, C5+, FMC 7+, Cyclin D1+, sIg+ (with IgM and IgD), and CD23-.
Cytogenetics:
The gold standard is the finding of the t (11;14).
Prognosis:
Very aggressive and usually resistant to therapy. Prognosis poor.
