What to Bring 

1.  You will need 5 changes of comfortable, loose-fitting non-restrictive clothing (sweat pants/loose T-shirts) that are easy to exercise in.  Patients need rubber soled shoes that tie (sneakers) for participation in therapy.

    a.  Clothing should be seasonal, as some activities are done outside               

    b.  All articles of clothing must be labeled with patients name

    c.  If you want to hang any clothes please bring hangers from home. Closets with a rod are available.

2.  Any corrective device or equipment (even if not used recently - eyeglasses, hearing aids, canes, etc) will be evaluated and adjusted as needed.  Suggestions for new equipment will be made as needed. Any hair dryer/curling irons etc need to be checked by our maintenance staff for safety.

3.  Copy of all healthcare decision forms (Advanced Directives, Living Will, etc.)

4.  List of home medications (including dose, strength, and time taken).                   

    DO NOT BRING MEDICATIONS from home.  Any medication (prescription or over the counter) must be administered by a rehabilitation nurse.

5.  Insurance card and driver's license.
