We would like your thoughts about how we address our diverse patient population at the UVa Health System.  Please read the questions below about your practice and circle an answer and provide any comments to help clarify.

1. I feel able to access and use the Cyracom phone system (for an Interpreter).
2. I am able to access and use the services of an Interpreter through Language Assistance Services.
3. I am able to locate and use Patient Education materials.
4. I am able to locate materials/services that have been adapted to meet the learning barriers of patients/families (audio recordings, magnifiers, lg print materials, materials written in other languages and literacy levels).
5. I know and follow the UVaHS policy about the use of family and friends as interpreters.
6. I understand and address my personal biases about various groups of patients.
7. I feel able to use the PNSO website (clinical resource portal) when I am looking for resources that will help me care for patients.
8. I assess the communication needs of my patients and adapt my teaching successfully.
9. I document the use of an Interpreter during communication with patients by noting the name, service, and ID number.
10. I communicate the patient's preferred communication method during hand-off of care.
11. I evaluate whether my patients understand what they have been told by having them repeat back the information.
