Required Logs

Family Medicine Clerkship students are required to keep two logs during their clerkship.
You will receive a blank copy of these logs during orientation. If you need another copy, click on the link for a .PDF file that you can print out).

Preceptor Log (PDF file)
The preceptor log is a blank schedule where you indicate how many half-days you worked with each physician.

Patient Log On the back page of the passport is a list which encompasses the essence of what you must experience during your four week rotation (including diagnoses, presenting complaints and preventive health).  For each of these cases, you must seek out involvement in the history, diagnosis, physical examination, and management of these patient encounters and document your level of participation.  Keep the tally on the paper passport, but enter these on-line under Family Medicine patient logs.  Even after you have completed the required number of cases, you should continue to log in your patient encounters for those diagnoses listed on the back page of the passport.
