Award: $500 gift to the charitable medical organization of the recipient's choice courtesy of Pfizer, Inc.

Purpose: Recognizes someone who has made a real difference in creating and nurturing a caring environment in health and disease prevention on behalf of either: the military and general public, the uninsured and the underserved, the international community, or service to the profession.

Who is eligible? Members of the Medical Society of Virginia or the Medical Society of Virginia Alliance who have demonstrated outstanding service to medicine, inspiring and promoting excellence in others.

The nomination packet: Nomination form and two support letters -- one from the nominator and one from another supporter (other than the nominee).
