Services Provided:

Evaluation and assessment of nutrition status
- Development and implementation of nutritional care plans and goals
- Weekly follow up for all patients to assess the effects of medical nutrition therapy (MNT)
- Education provided to patients and family members to optimize their recovery and their nutritional and medical outcomes
Goals of Nutrition Therapy:
- To complete and utilize a nutrition focused assessment
- Guide and support patients through treatment
- Provide education to patients and family members
- Help individuals learn the self management skills to successfully manage diet related risk factors and symptoms of disease and their treatments in an evidence-based, team orientented manner
