Where are Services Available? Continuum staff can visit patients in their homes to provide care in eleven counties within Central Virginia: Albemarle, Greene, Madison, Orange, Louisa, Fluvanna, Buckingham, Nelson, Augusta, and Rockingham. Additionally, Continuum can prepare and deliver home infusion products and supplies throughout Virginia and into some neighboring states. In instances where the patient lives outside of Continuum's eleven county direct service area, Continuum will work with the patient to identify and select a Home Health provider where the patient lives to provide the at-home care.
