What is Continuum?
The UVa Health System has its own home health agency, Continuum Home Health Care (CHHC), which provides healthcare for patients who require care at home.
The services are provided by qualified nurses, pharmacists, therapists, social workers, nutritionists, aides and companions. Continuum can provide prescribed medications, nutritional products, and home infusion equipment if needed by patients to complete their care at home.
Continuum is under the operational direction of the Health System Chief Clinical Officer and carries out its day-to-day functions under the direction of an Agency Director. The clinical governing board of the University of Virginia Medical Center, the Clinical Staff Executive Committee, provides clinical oversight.
Continuum is Medicare and Medicaid certified as a home health agency by the Commonwealth of Virginia Department of Health.
Continuum is accredited by JCAHO (Joint Commission Accreditation Healthcare Organization).
