Instructions:   This application is for one of the two CCEP Fellowships beginning in July, 2011.  Please note the newly revised deadline for applications: October 30, 2009.   We will conduct interviews in December, 2009 and January, 2010.

3. Please arrange for  two letters of recommendation to be sent to Michael Mangrum, MD, Director of Clinical Cardiac Electrophysiology. One letter must come from your Cardiovascular or Cardiology Fellowship Program Director and a second letter from the Division Chief of Cardiology at your present institution. Letters must be individually signed and sent in sealed envelopes, preferably mailed directly from the recommender's office.   Additional letters of reference are welcome.  All letters should be mailed to:
