The Care Connection for Children Program can help your child if he/she meets the following requirements:

  • a resident of the Commonwealth of Virginia
  • under the age of 21
  • has special health care needs that are not usually needed by children of the same age.
  • has a physical basis

HOW TO APPLY:                                       is that real

A parent, legal guardian or a health care provider can call our Center (1-866-596-9367) to make a referral and to determine if the child is eligible for the program.  There is no annual fee.  There is no cost to the family for care coordination.  In order to be eligible for financial assistance through the program the family income must be below 300% of the federal poverty level.

El padre, encargado legal, o el doctor pueden contactar nuestro Centro (1-800-596-9367) para hacer un referido y determinar si el nino es eligible para el progarma. No tiene que pagar una cuota anual. La coordinacion de cuidado es gratis. Para poder beneficiar de asistencia financial por medio del programa, el ingreso de la familia debe ser debajo de 300% del nivel de povreza federal.
