Sample submission forms and protocols

Please include a sample submission form when bringing samples or a synthesis request to the Biomolecular Research Facility.

These forms require Adobe Acrobat Viewer to be read.

Information on samples for mass spectrometry analysis (sequencing, mass determination) should be entered in the Proteus LIMS system (see Getting started for a brief introduction)

peptide synthesis form
The peptide synthesis form has fields which can be filled on your computer and then printed; the page cannot be saved or mailed. Alternatively you can print the form and fill in data by hand. The fields are fixed length, so characters which do not show on your screen will not print and thus not be recorded.
Move between fields with the Tab key or by clicking with a mouse.
For check boxes, move the cursor until you see an bar cursor, rather than a hand, and click.
After filling the last field on the page, move the cursor out of the field by clicking somewhere else and print- trying to save the page does not work.
