    ICU’s/OR/Procedural   - (ABG/VBG)

    Verbal or EPIC order should be obtained prior to collection of POCT

    Orders are needed to ensure what specific tests should be obtained   and to prevent unnecessary tests.

    gather all   needed supplies to obtain POCT 

    Obtain correct POCT supplies

    -Unit specific locations 

    Gathering needed supplies ensures you have the tools you need to   complete the task correctly the 1st time.

    Obtain patient chart label        

    Obtain patient label prior to collecting   POCT 

    ICU’s- labels in patient room

    ED/OR/Procedural-From patient chart 

    Specimen must be labeled in the presence of the patient

    Patient Identification 

    Double verification   with other POCT competent staff (VBG/ABG). 

    ED-Single Verification for Urine sample.    

    Verify patient’s Name and   MRN on the POCT sample and patient ID armband match

    All sample collections must be labeled in   the presence of the patient.     

    Double Verification prevents labeling errors and inadvertent   treatment to incorrect patients with VBG/ABG samples.

     

    Location of Labels

    ICU’s (Adult, PICU, NICU*)

      

    • Upon patient transfer/discharge from ICU room, RN,PCA/T, or HUC will appropriately dispose of patient labels according to hospital policy 

    ED/OR/Procedural Areas

    • Labels will be in patients chart
    • Labels should remain in chart and follow patient to next in-house destination
    • If discharged-labels should be appropriately disposed of according to hospital policy
