policy 0197 : Suicide Risk Assessment and Prevention

All patients twelve (12) years of age or older in the emergency department, inpatient setting, or psychiatric outpatient clinic who are being evaluated or treated for behavioral health conditions as their primary reason for care shall be screened for risk of suicide using a validated tool. When indicated based on the results of the screening, patients shall be assessed for level of risk and risk stratification. Patients shall be screened, assessed and cared for as outlined in this policy.
