policy 0324 : Clinical Communication and Escalation of Care/Inpatient Services

Health care providers at the University of Virginia Medical Center Medical Center are responsible for their own actions and omissions in administering care to patients, and for providing medical attention for patients when needed. Clear, consistent and structured communication among members of health care teams is required to properly administer, coordinate and manage patient care; and promptly and appropriately address any change in a patient’s condition. This policy defines the clinical communication expectations for health care team members in the inpatient setting when patient care must be escalated.
