policy 0025 : Discharge Planning

Discharge planning is a continuous, multidisciplinary process applied to all patients admitted to the University of Virginia Medical Center, including outpatients receiving sedation or anesthesia, regardless of length of stay. Discharge planning shall begin upon admission and a safe discharge plan shall be in place prior to discharge. Discharge shall be based on the determination that the patient is medically ready for discharge.
