Thank you for your interest in our programs. You may elect to fill out our Information Request Form, or you can procede to apply online (keep reading for instructions). You may also download copies of our UVa CPE application and Letter of Reference Form to print off and fill in by hand (Adobe Acrobat Reader is required). Our programs are offered on a space available basis, so we encourage you to call first before applying to make sure there is space. Applications for Levels I and II CPE are accessible by clicking the links on the right side of this page. (Prospective international exchange students should click here for more information.) All applicants must fill out a UVa CPE application form. Our application form differs slightly from the standard ACPE, Inc. form. Please use our form and observe our application requirements. If you choose to submit the form online, you still need to send the written portion of your application through the US mail. Please print a copy of the online form before you submit. You will need to sign this copy and send it, with your materials and non-refundable $50 processing fee, to:
